Rail workers four seconds away from being hit by 98mph train
Investigation into near miss found incident was largely caused by human error, as group had no warning of approaching train from colleague on lookout
Nine rail workers had a brush with death after a passenger train sped past them at almost 100mph as they worked on a section of track.
The workers were repairing a section of the west coast mainline in Lancashire when the near miss occurred on 22 September last year.
